Dear Parents,
Welcome to Bright Beginnings Co-operative Preschool! The following is a list and guide to the necessary forms for your child’s enrollment in Bright Beginnings Preschool. All of the forms are attached. These forms must be turned in at the June membership meeting. In addition to the forms, there are several pages of information for you to keep: Tuition Schedule, School Calendar, Committee Descriptions, and Executive Board Descriptions. Please contact the registrar if you have questions.

• Application: There are important authorizations on this page so please read carefully.
• Parent Agreement Form: Needs both parents’ signature (if applicable). Refers to the Parents Handbook, Constitution and By-Laws. Each family will receive the Handbook before school starts. The Handbook needs to be returned at the end of the academic year to receive the $5 deposit back.
• Child Enrollment and Health Information (also know as ETA–Emergency Transit Authorization). This is a very important form! Your child cannot be left at preschool without this form completely filled out. Fill out mother and father’s information separately, include all addresses and phone numbers. Even if your child does not see a dentist regularly, a dentist with address and phone number needs to be listed in case of emergency. On the next page, please sign either permission to transport or not (and indicate actions you want taken). Write in all immunizations as the Child Medical Statement will not be available during field trips or emergencies. Please indicate any allergies, esp. food allergies so that snacks can be altered as needed. If you child has a medical condition that may occasionally require medication (ie. a nebulizer or inhaler for asthma, an Epi-pen for allergies) please ask the registrar for forms that are required by the state to administer medications.
• Preference Form: Assists the Vice President in scheduling parent work days and your child’s special day. Please read the Committee Descriptions and the Executive Board Descriptions to assist you in filling out this form.
• Child Medical Statement: To be filled out and signed by your child’s physician. These forms are good for one calendar year only.
• Certification of Compliance/Photo Permission Form: The Certification of Compliance portion ensures that those adults transporting students during field trips are insured.
• Drop-off/Pick-up Authorization: List persons whom you authorize to drop off or pick up your child from preschool. A photo ID will be needed from non-parents.
• Child Information: Assist the teacher in understanding your child’s interests.
• Statement of Non-conviction: Each adult who will be assisting in the classroom needs to fill out top half of the form. The bottom of the form is for teachers and substitute teachers. Let the registrar know if your family needs more than one form.
• Second Adult Statement: Each adult who will be assisting in the classroom needs to fill out this form. Write in 9:00 a.m. to 12:30 p.m. for the time slots and MWF.
